Prior to your visit:
Forms completed: Please arrive 20 minutes prior to your scheduled appointment if you have already completed the downloaded forms.
Forms not completed: Please arrive 35 minutes prior to your scheduled appointment if your have not completed these forms.
During your visit:
Provide current insurance cards. If you have no insurance coverage, a minimum of $350 will be collected at the time of appointment. Your driver’s license or other photo ID will be scanned for your file to protect your identity.
Please bring x-rays, test results, and appropriate medical records from third-party physicians. This will ensure you will be appropriately treated. Bighorn Medical Center cannot guarantee receipt in time for your visit if provided by third parties.
A list of medications and herbal supplements with dosages should be provided at the time of your visit. In addition, a list of your allergies along with typical reactions will be required.
Completed forms are required for all new patients and updated forms will be required each calendar year for all existing patients. Insurance co-payments for your visit as required by your insurance will also be required during your visit. We accept cash, check, MasterCard, Visa and Discover and American Express.
Provide a copy of the first report of your injury including your claim number during your visit. You will also need to provide your health insurance information. Doing so will protect you if workers’ compensation provider contests or denies your claim. We can, in those circumstances, bill your health insurance provider. Otherwise, service charges become the responsibility of the patient. If your workers’ compensation provider does not contest or deny your claim for your work-related injury then Bighorn Medical Center will not use your health insurance information. We collect health insurance information in these cases for the protection of our patients.
